Job Summary:The PRN LVN provides skilled nursing care to patients in their homes on an as‑needed basis. This role focuses on direct patient care, clinical assessments, care planning, and timely documentation. The PRN LVN collaborates with the Supervising RN, therapy team, intake/admissions, and scheduling to ensure coordinated and high‑quality patient care. This position does not include supervisory or performance‑management responsibilities.Duties/Responsibilities:
- Provide skilled nursing visits in accordance with agency policy and patient care plans.
- Conduct ongoing assessments of the patient’s condition, needs, and home environment.
- Document all patient interactions, assessments, interventions, and communications in the Electronic Medical Record accurately and timely.
- Communicate changes in patient condition to the Supervising RN, physician, and care team as appropriate.
- Collaborate with intake/admissions, therapy services (PT/OT), and scheduling to support appropriate care delivery.
- Ensure compliance with agency policies, Medicare requirements, and Texas home‑health regulations.
- Participate in on‑call rotation or respond to after‑hours case escalations as needed (within PRN availability).
- Provide patient and caregiver education related to disease processes, medications, safety, and care needed.
- Complete other patient‑care‑related assignments as requested and assigned.
Required Skills/Abilities:
- Current Texas LVN license.
- Minimum 3 years of LVN experience, with at least 1–2 years in home health or community‑based care preferred.
- OASIS experience strongly preferred.
- Valid driver’s license and reliable transportation with current insurance for field visits.
- Strong documentation and electronic charting skills.
- Excellent verbal, interpersonal, customer service, and written communication skills.
- Strong organizational skills and attention to detail.
- Effective time‑management skills with the ability to meet visit and documentation timelines.
- Strong clinical judgment and problem‑solving abilities.
Physical Requirements: Must be able to be fingerprinted and TB tested or provide proof of a negative TB test within the last 12 months prior to hire. Prolonged periods of sitting and computer work may be required. Must be able to lift up to 50 pounds as needed.
- Travel required up to or exceeding 85% depending on caseload and PRN availability.
Pay: $30.00 per visit
