Job Summary: As an RN/LPN in our Memory Care Unit, you'll provide skilled nursing care under the direction of the attending physician and within the scope of state nursing practice guidelines. You will play a key role in ensuring each resident receives individualized, compassionate care.
Responsibilities of Registered Nurses (RN) / Licensed Practical Nurses (LPN):
- Monitor and assess residents for changes in condition
- Administer medications and treatments per provider orders
- Provide support with ADLs and personal care
- Oversee and support CNAs
- Maintain accurate documentation and charting
- Collaborate with interdisciplinary teams to create care plans
- Provide trauma-informed and culturally sensitive care
- Participate in admissions, discharges, and care plan reviews
- Maintain compliance with infection control, safety, and facility policies
- Valid RN or LPN license in the state of Illinois
- CPR certification required
- Compassion, patience, and a strong work ethic
- Ability to work independently and as part of a team
