Physical Therapist - Home Health

Location: Santa Maria, CA

Start Date: 04-27-2026

Shift Type: 8:00 AM - 5:00 PM 

Hours per Week: 40

Roles & Responsibilities

• Perform thorough home-based patient assessments and create individualized PT treatment plans
• Deliver skilled therapeutic interventions for orthopedic, stroke, joint and spinal injury patients
• Complete accurate, timely documentation using Cerner and OASIS as required
• Collaborate with OT, Speech, OTA/PTA and rehab aides to coordinate patient care
• Educate patients and families on home exercise programs, safety and recovery goals
• Supervise and mentor rehab aides, OTA/PTA when appropriate to ensure quality care
• Participate in interdisciplinary communication and case management to optimize outcomes
• Maintain professional standards including BLS certification and adherence to facility policies

Knowledge & Skills

• Strong knowledge of home health workflows and OASIS requirements
• Skilled in assessment and treatment of orthopedics, joint rehab, stroke and spinal injuries
• Proficient in Cerner charting and timely documentation
• Excellent patient and family education skills
• Ability to collaborate effectively with OT, Speech, OTA/PTA and interdisciplinary teams
• Good clinical judgment, time management and organizational skills for field-based care

Requirements

• Minimum 2 years of clinical experience (job posting lists 2 years); posting also notes at least 1 year working PT/OT/SLP
• Experience with OASIS documentation is required
• Familiarity with Cerner EMR for charting and documentation
• BLS certification required
• Home health experience preferred but not required
• Comfort working in a geographically assigned home health region and conducting in-home visits
• Speech candidates: CCC-SLP or CCC-A credentials noted as preferred/considered
