Job Description:
The Social Worker is responsible for acute care case management, healthcare financial and reimbursement planning, post-discharge planning, and providing resources for post-discharge, using EPIC systems.
Responsibilities:
- Provide acute care case management for inpatient adults, including ICU, PCU, and MedSurg patients.
- Conduct post-discharge planning and provide resources.
Requirements:
- 2 years of recent acute inpatient case management experience required.
- 4 years of overall nursing experience required.
- EPIC experience required.
- ADN or BSN (or above) accepted.
