What are the responsibilities and job description for the RN, Patient Onboarding Nurse (FT, PT, Call-In; 12pm-8:30pm) position at Allied Services?
Brief Description
Under the direction of the Heinz Transitional Director of Nursing, the Patient Onboarding Nurse coordinates the new admission’s transition into Heinz Transitional Unit. This position facilitates a positive first impression to the new patient, and their representative, if applicable, through facility and staff introductions, orientation and utilization of strong customer service skills during this transition period. This position ensures timely review and coordination of physician orders with entry into the EMR system, nursing assessment coordination and implementation of any required interventions needed for the patient’s baseline care plan.
The Patient Onboarding Nurse must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served within the Nursing Department – adult through geriatric. This position must demonstrate knowledge of the principles of growth and development over the life span; coordinate the assessment of data reflective of the patient's status and to provide the care needed as described in the Nursing Department's policies and procedures.

EDUCATION:
Graduate of an accredited Nursing Program required.
WORK EXPERIENCE:
Minimum of one (1) year of experience preferred as RN/LPN preferred.
SPECIAL EMPLOYMENT REQUIREMENTS:
Possession of a current license as a Registered Nurse/LPN as issued by the Pennsylvania State Board of Nurse Examiners.
Ability to interact and communicate effectively both verbally and in writing required.
Proficient in data entry into the EMR system.
Strong time management skills.
CPR certification must be obtained and maintained.
Must be cleared by PA State Police criminal history check
Must be able to demonstrate the knowledge and skills necessary to provide individualized care appropriate to the age specific need and culture of the patient served.
Participation in formal staff development programs.
