Allied Home Health Care and Nursing Services, LLC is a physical therapist owned skilled home health care agency based out of Detroit, Michigan. This company was created for the sole purpose of providing top quality care to patients and making them feel comfortable and supported. Our Director of Nursing has been with the company since 2017 and is currently finishing her DNP.
We are looking for a registered nurse who is a motivated learner, a team player, and is looking for a part time or full time position with Allied.
JOB SUMMARY
A registered nurse administers skilled nursing Care to patients on an intermittent basis in their place of residence. This is performed in accordance with the physician orders and plan of care under the direction and supervision of the Director of clinical services/Nursing supervisor.
Qualifications
- Graduate of an approved school of professional nursing in in currently licensed and the state(s) in which practicing two years nursing experience, preferred
- Acceptance of philosophy and the goals of this agency
- Ability to exercise initiative and Independent judgment
Responsibilities
- Provide services in accordance with a plan of care
- Makes the initial evaluation visit and regularly reevaluate the patients nursing needs
- Initiates the plan of care and necessary revisions
- Provide those services required substantial specialized nursing skills
- Initiate appropriate preventative and rehabilitative nursing procedures
- Prepare clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per agency policy
- Coordinate services
- Informs personnel changesIn that condition and needs of the patient
- Canceled the patient and family/significant others in meeting nursing and related needs
- Participate in and present in service programs
- Understands and appears to establish agency policies and procedures
- Processes orders and notify position of patient needs and changes in condition. Complete certification/recertification orders and discharge summaries.
- Determines the amount and type of nursing needed by each individual patient
- Refers to physical therapist, speech language pathologist, Occupational therapist and medical social worker those patients requiring their specialized skills
- Supervises and teaches other nursing personnel
- Conducts patient care conferences on patients assigned to his/her care
- Participates in pure review and performance improvement as assigned.
- Participate in utilization review of medical records as assigned
- Take on call duty nights, weekends, and holidays, as assigned
- Complete and submit oasis assessments, reassessments transfers, resumption of care, discharges, in significant change in condition, in accordance with agency defined time frames
- Appropriately utilizes ICD – nine codes
Working environment
- Works indoors in agency office and patient homes and travels to/from patient homes
Job relationships
- Supervised by: Director of clinical services/nursing supervisor
- Worker supervised: Licensed practical nurse, home health aide
Lifting Requirements
- Ability to perform the following tasks if necessary:
- Ability to participate in physical Activity
- Ability to work for extended period of time while standing and being involved in physical activity
- Ability to do extensive bending, lifting, and standing on a regular basis.

- $60.00 - $100.00 per hour
- Orientation Period and In Services: $25.00-$30.00 per hour
