Duties
- Assist clients with daily living activities such as bathing, dressing, grooming, and mobility support to promote independence and safety.
- Provide companionship and emotional support to enhance clients’ mental well-being and reduce feelings of isolation.
- Administer medication reminders and monitor health status, reporting any changes or concerns to healthcare professionals promptly.
- Support clients with light housekeeping tasks, meal preparation, and transportation to appointments or errands.
- Follow individualized care plans meticulously to ensure all client needs are met with compassion and professionalism.
- Maintain accurate documentation of care provided, observations, and any incidents in compliance with agency policies.
- Collaborate with family members and healthcare teams to ensure consistent, high-quality care tailored to each client’s unique needs.
Experience
- Prior experience in home care or personal caregiving is highly preferred.
- Experience working with diverse populations including seniors, individuals with chronic illnesses, or post-acute care patients.
- Knowledge of medical-surgical procedures, geriatrics, emergency response protocols, and post-acute recovery processes.
- Familiarity with Level II trauma center protocols or hospital medicine environments is advantageous.
- Experience supporting clients in outpatient clinics or sub-acute care settings demonstrates versatility in care delivery.
- Strong interpersonal skills combined with the ability to follow detailed care instructions accurately are essential for success in this role.
