Job Summary
Allied Healthcare Physicians PLLC is seeking a compassionate and dedicated Adult Nurse Practitioner (ANP) to join our growing team. This role is ideal for an experienced clinician who is passionate about providing high-quality, patient-centered care in a collaborative and supportive environment.
As an Adult Nurse Practitioner, you will play a key role in managing the health and wellness of adult patients, focusing on preventive care, chronic disease management, and comprehensive treatment planning.
Key Responsibilities
1. Patient Assessment & Evaluation
- Conduct thorough initial and follow-up assessments, including complete medical histories and physical examinations.
- Identify symptoms, order and interpret diagnostic tests, and establish accurate differential diagnoses.
2. Care Planning & Management
- Develop and implement individualized treatment plans tailored to each patient’s health goals and clinical needs.
- Prescribe medications, manage ongoing therapies, and coordinate referrals to specialists when necessary.
- Monitor patient progress and adjust care plans to ensure optimal outcomes.
3. Patient Education & Health Promotion
- Educate patients and families about diagnoses, treatment options, and preventive care.
- Provide counseling on nutrition, exercise, stress management, and medication adherence.
4. Clinical Procedures
- Perform routine clinical tasks, including wound care, injections, and minor procedures.
- Ensure proper documentation and follow-up for all interventions performed.
5. Care Coordination
- Collaborate closely with physicians, nurses, and multidisciplinary team members to deliver seamless, integrated care.
- Communicate effectively with patients and families regarding treatment goals and progress.
6. Documentation & Compliance
- Maintain accurate, timely electronic health records (EHR).
- Ensure all documentation complies with regulatory, legal, and organizational standards.
7. Quality & Professional Development
- Participate in quality improvement initiatives and adhere to best-practice clinical guidelines.
- Stay current with advances in adult primary care through ongoing education and training.
8. Administrative Support
- Manage patient follow-ups, messages, and clinical inquiries.
- Support efficient patient flow through scheduling and triage coordination.
9. Safety & Regulatory Compliance
- Follow all state and federal healthcare regulations, infection control policies, and patient confidentiality standards.
Qualifications
- Current and unrestricted Nurse Practitioner license in New York.
- Minimum 1 year of clinical experience in adult or primary care preferred.
- Strong interpersonal, communication, and clinical decision-making skills.
- Proficiency in Electronic Health Records (EHR) systems.
Job Types: Full-time, Part-time
Pay: $120,000.00 - $140,000.00 per year
Expected hours: 30 – 40 per week
