We are seeking a highly skilled and adaptable Nurse Practitioner (NP) to join our dynamic healthcare team in a field-based capacity. The ideal candidate will possess extensive clinical expertise, including experience with acute and chronic conditions, telehealth services, and specialized procedures such as suturing, IV infusion, and catheterization. Familiarity with EMR/EHR systems like Epic, Cerner, and eClinicalWorks is essential. This position emphasizes delivering high-quality patient care while working within multidisciplinary teams to improve health outcomes.
Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history documentation using EMR/EHR systems such as Epic and Athenahealth.
- Manage acute and chronic conditions across various specialties including geriatrics, pediatrics, internal medicine, emergency medicine, behavioral health, and infectious disease care.
- Perform advanced procedures such as suturing, IV insertion and infusion, catheterization, spinal taps, gastric lavage, and ventilator management.
- Provide telehealth consultations utilizing telemedicine platforms to extend care access and coordinate follow-up treatments remotely.
- Assist in diagnostic evaluations through laboratory testing, specimen collection/processing, medical imaging interpretation (including sonography), and diagnostic evaluation techniques.
- Develop treatment plans incorporating medication administration, wound care (including sterile processing), pain management strategies—including acute pain management—and post-acute care planning.
- Collaborate with specialists in areas such as cardiology (cardiac catheterization), ophthalmology (ophthalmology exams), anesthesiology (airway management), and other disciplines to ensure comprehensive patient care.
- Educate patients on health coaching topics such as nutrition, family planning, dementia care, Alzheimer's care, memory care, and working with people with disabilities or developmental disabilities.
- Ensure compliance with HIPAA regulations and maintain accurate medical documentation including CPT coding, ICD-9/10 coding, and medical records management.
- Participate in discharge planning and utilization review to optimize patient outcomes across inpatient and outpatient settings.
- Engage in clinical research initiatives or quality improvement projects to advance practice standards within the organization.
Experience
- Proven clinical experience in hospital medicine or Level I/II trauma centers; ICU or PICU experience preferred.
- Demonstrated expertise in emergency room procedures such as airway management and ventilator support; experience with acute pain management is advantageous.
- Strong background working with EMR/EHR systems like Epic, Cerner, eClinicalWorks; proficiency in medical documentation and coding practices including CPT and ICD coding is required.
- Experience providing care in diverse environments such as nursing homes, hospice & palliative medicine settings, assisted living facilities, or home health is highly desirable.
- Familiarity with telehealth platforms for remote patient assessments; experience working within NCQA standards or utilization review processes is a plus.
- Ability to perform specialized procedures including dermal fillers or esthetic laser treatments is a bonus but not mandatory.
- Knowledge of infection control protocols—including aseptic technique—and familiarity with infection prevention measures are essential for ensuring patient safety across all settings.
