  • $25–$40/hour depending on experience

Program Expansion Opportunity

Allied Community Services is seeking an experienced Targeted Case Manager to provide services in the Lewiston/Auburn area.

This role is ideal for someone who enjoys working closely with families, coordinating services across systems, and helping build strong community partnerships. There is significant opportunity for professional growth and future leadership as the program expands.

Key Responsibilities

  • Provide community-based case management for adults with behavioral health conditions, developmental disabilities, or chronic medical conditions
  • Conduct strengths and needs assessments
  • Develop and maintain Individual Service Plans (ISPs) and review every 90 days
  • Coordinate services with behavioral health providers, medical providers, and community organizations
  • Facilitate and participate in treatment team meetings
  • Connect families to appropriate services and supports
  • Monitor progress and adjust services as needed
  • Maintain timely and accurate documentation in compliance with company requirements
  • Maintain regular contact with families through home visits, community meetings, and telehealth

Minimum Qualifications

Applicants must meet one of the following qualifications:

  • 1. Bachelor’s degree in a related field such as Social Work, Psychology, Behavioral Health, Counseling, etc

OR

  • 2. Bachelor’s degree in an unrelated field AND at least one year of relevant human services experience.

Preferred Qualifications

  • Experience working with adults in a behavioral health setting
  • Knowledge of behavioral health services within the Maine community
  • Experience coordinating services across healthcare and community providers
  • MHRT/C certification (or willingness to obtain—paid training provided)

Requirements

  • Valid Maine driver’s license
  • Reliable transportation
  • Active auto insurance
  • Ability to travel locally for home visits and community meetings
  • Basic proficiency with Microsoft Office (Outlook, Word, Excel)
  • Ability to learn and use electronic documentation and case management systems
