Job Description
JOB TITLE: Case Worker DEPT: Supportive Services
REPORTS TO: Program Coordinator FLSA: Non-exempt
SALARY RANGE: Grade 6
Position Summary:
The Caseworker supports individuals and families by connecting them to essential services and resources that promote self-sufficiency and well-being. This role involves direct client engagement, case management, and collaboration with community partners to ensure individuals receive the support they need.
Key Responsibilities:
• Conduct client interviews, assess eligibility, and enroll individuals and families in agency programs.
• Assist clients in developing action plans to improve their circumstances and achieve self-sufficiency.
• Provide referrals and follow up with community resources to address clients’ social service and health needs.
• Collaborate with clients to create and implement personalized action plans with clear goals.
• Support clients in achieving economic independence through goal-oriented planning.
• Conduct follow-ups with clients at least every six months and annually to monitor progress.
• Advocate on behalf of individuals and families to ensure they receive the necessary support.
• Utilize CAPTAINS case management and other required software proficiently.
• Participate in interdisciplinary case conferences to coordinate services effectively.
• Maintain knowledge of local social service and health agencies, fostering strong referral partnerships.
• Develop and maintain a community resource database for client support.
• Prepare reports and enter client data accurately into the system.
• Perform other related duties as assigned.
Qualifications:
• Education & Experience:
o Bachelor's degree in Human Services or a related field with at least one year of experience in human services.
o OR Associate degree in Human Services or related field with at least two years of experience.
o OR High School diploma with three years of experience in human services.
• Knowledge of social service and health agencies in the Greater Bridgeport area.
• Strong verbal, written, and interpersonal communication skills.
• Bilingual communication skills (oral and written) are a plus but not required.
• Proficiency in computer use, including case management systems.
