What are the responsibilities and job description for the Hospice Liaison position at Alleviate Care?
Job Summary
The Hospice Liaison is required to communicate Hospice services, value and philosophy to the medical community, physicians, patients and families in a manner that results in increasing referral sources and admission results. This is managed through pro-active and continual business development and successful admission results of hospice appropriate patients. Must be able to establish, build and maintain positive relationships with customer and referral sources while responding to customer requests and concerns within assigned area.
Responsibilities and Duties
1. Coordinates referral process in conjunction with Admissions Department
2. Ensures compliance with all state federal, and JACOH referral/intake regulatory requirements
3. Establishes and maintains positive working relationships with current and potential referral sources.
4. Builds and monitors community and customer perceptions of Hospice as a high quality provider of services
5. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should service not be provided by Hospice.
6. Must enforce and follow all safe practices in the performance of job duties
7. Understands and maintains HIPPA guidelines for patient confidentiality
8. Facilitates Hospice referral process, serves as a resource to review and consult available patient information related to the case, to determine hospice care needs and criteria; coordinates and communicates with Hospice agency leadership and referral sources to promote care coordination and a timely admission process.
9. Ability to build and maintain positive work relationships both internally and externally
Job Type: Full-time
Pay: $75,000.00 - $100,000.00 per year
