What are the responsibilities and job description for the Hospice CHHA position at Alleviate Care?
Job Summary:
Performs personal care and assists the patient in activities of daily living under direction of a registered nurse. The Home Health Aide assignment shall be made in accordance to the Plan of Treatment.
Qualifications:
Completion of a California Department of Health approved training program as outlined in Section 74747 of Title 22 or its equivalent/
Job Knowledge:
Responsibilities and Duties:
- Assists patients with personal hygiene such as skin, mouth, hair care and bathing.
- Assists patients in and out of bed and assists with ambulation.
- Assists with prescribed exercises which patients and aides have been taught by appropriate health personnel.
- Prepares meals, including therapeutic diets, and assists patients with eating.
- Assists patients to the bathroom or in using commodes, bedpans or urinals.
- Performs household services which will facilitate the patient’s self-care at home and are necessary to prevent or postpone institutionalization.
- Assists patients with medications which are ordinarily self-administered. The home health aide shall not administer medications of any kind.
- Reports changes in the patient’s condition and needs to the supervising nurse or therapist.
- Completes records regarding services performed.

Pay: $24.00 - $27.00 per hour
