Licensed Practical Nurse (LPN) [PRN]

Responsibilities: 

• Participates in the delivery of care as set by the care plan created by the RN Case Manager and the interdisciplinary team.  

• Assists with planning and implementing ongoing audit reimbursement program in accordance with federal, state, and Hospice standards, under the supervision of a registered nurse.  

• Willingness to be flexible and agile as a field LPN. 

• Reports findings and concerns to the RN Case Manager. 

• Audits one complete chart for each field nurse in the QAPI team monthly.  

• Possesses adequate knowledge of Medicare/Medicaid hospice benefit and insurance standards to ensure reimbursement for all services rendered and the regulatory standard of care is provided. 

• Makes visits as scheduled 

• Maintains patient rights and dignity. Assures the care of the dying patient, optimizing his/her comfort and dignity.

Requirements:  

• Successful completion of LPN or LVN training program 

• Experience: 1 year related  

• Previous hospice or home health experience a plus 

• Current LPN license; driver’s license and current automobile liability insurance

Salary : $27 - $29
