Primary Responsibilities
- Welcome referrals from pediatricians into the behavioral health program and provide a steady, supportive point of contact for patients and families
- Support informed consent and help families understand the program, next steps, and their role in follow-through
- Coordinate intake, scheduling, referrals, follow-up, and communication via phone and/or email to support integrated care within the child's medical home
- Maintain patient registries, track outreach and enrollment, and keep documentation timely and accurate
- Identify urgent behavioral health concerns and support triage through established workflows
- Guide families to appropriate community behavioral health resources and support warm handoffs when available
- Maintain referral resources and strengthen community partnerships that support access to care
- Contribute to quality improvement by identifying workflow gaps and supporting program refinement
- Excellent communication and emotional intelligence, with a calm, compassionate approach to families, colleagues, and community partners
- Ability to handle difficult conversations with empathy, sound judgment, and professionalism, especially during times of crisis
- Strength in building trusting relationships while maintaining appropriate professional boundaries
- Strong organization, reliability, and follow-through in managing multiple tasks and communication details
- A proactive mindset and interest in improving workflows and strengthening the program
- Curiosity, willingness to ask questions, and enthusiasm for learning quickly in a rapidly changing field
- Ability to work independently and collaboratively as part of a team
- Experience in healthcare, pediatrics, mental health, or care coordination is appreciated but not required
- Working knowledge of Microsoft 365 (Outlook, Word, Excel, Edge), Adobe, MS Teams, and Zoom
- Experience with an EHR system and willingness to learn Genesys (phones), ProviderFlow (eFax), Phreesia (patient engagement), and Greenway Intergy (EMR)
- Ability to learn new technical programs quickly
- Familiarity with Dropbox preferred
- Understanding of HIPAA
- Enjoy working in a fast-paced, patient-focused environment with ongoing changes
- Must have reliable transportation and be willing to travel to all clinics as staffing needs change
- Must be 18 years of age or older

- All employees are currently required to wear a surgical, KN95, or N95 mask at all times while in clinic and patient-facing
- High School diploma/GED or higher
- Additional coursework or experience in healthcare, behavioral health, child development, psychology, social services, or care coordination preferred
- 25–30 hours per week, Monday through Friday
- Approximately 5–6 hours per day between 9:00 AM and 3:00 PM
- Following initial in-office training, this role will transition to a hybrid schedule
