We are seeking a compassionate, dependable Certified Nursing Assistant (CNA) to join our growing hospice team. In this role, you will provide hands-on personal care and emotional support to hospice patients in skilled nursing facilities, assisted living communities, and private residences throughout our service area.
Responsibilities
Provide personal care including bathing, grooming, dressing, toileting, oral care, and skin care.
Follow each patient's individualized plan of care.
Observe, document, and report changes in patient condition to the RN Case Manager.
Accurately complete visit documentation in a timely manner.
Maintain infection prevention and safety standards.
Collaborate with the interdisciplinary hospice team to provide exceptional end-of-life care.
Support patients and families with dignity, compassion, and respect.
Qualifications
Current Wisconsin CNA certification.
Valid driver's license, reliable vehicle, and current automobile insurance.
Ability and willingness to travel throughout Allegiant Hospice's service area.
Strong communication and interpersonal skills.
Hospice, home health, or long-term care experience preferred but not required.
Willingness to travel between patient locations, including skilled nursing facilities, assisted living communities, and private residences.
