Clinical Manager (RN) Adult & Geriatric Home Care (Licensed Home Care Services Agency LHCSA)
Location: Suffolk County, NY Full-Time and Part-Time
Reports To: Executive Director
FLSA Status: Exempt, Full-Time
Position Summary
The Clinical Manager is responsible for overseeing and coordinating all clinical aspects of home care services provided to adult and geriatric clients. This position ensures that all client care is delivered in compliance with regulatory standards, clinical best practices, and organizational policies. The Clinical Manager provides leadership and supervision to field nurses and caregivers, promotes client satisfaction, and supports successful aging in place through safe, compassionate, and high-quality care in the home environment.
Essential Duties And Responsibilities
Clinical Oversight & Supervision
Oversee and manage the clinical delivery of home care services to adult and senior clients.
Conduct in-home client assessments and develop individualized care plans in collaboration with clients, families, and interdisciplinary team members.
Supervise and evaluate field nurses and caregivers to ensure compliance with care plans and regulatory standards.
Provide clinical guidance and support to staff in addressing complex client needs or changes in condition.
Participate in the on-call rotation to provide 24/7 clinical support as needed.
Documentation & Compliance
Maintain accurate, timely, and complete clinical documentation in the electronic medical record (EMR) and client home charts.
Monitor compliance with state and federal regulations, agency policies, and LHCSA standards.
Review and approve clinical documentation and care plans for completeness and accuracy.
Ensure all client and staff records meet DOH and HIPAA confidentiality requirements.
Training & Development
Identify training needs and provide ongoing education to caregivers and nursing staff.
Facilitate orientation and competency evaluations for new clinical employees.
Support a culture of professional growth, continuous learning, and quality improvement.
Collaboration & Communication
Serve as a liaison among clients, families, caregivers, physicians, and community partners.
Communicate promptly regarding changes in client status, care needs, or emergency situations.
Promote effective teamwork and a positive work environment among all staff.
Required Qualifications
Graduate of an accredited nursing program with an active, unrestricted Registered Nurse (RN) license in New York State.
Bachelor of Science in Nursing (BSN) preferred.
Minimum two (2) years of nursing experience, including at least one year in home care, home health, rehabilitation, or case management.
Demonstrated expertise in clinical assessment, critical thinking, and care coordination.
Current CPR certification; CPR Instructor certification preferred.
Excellent communication, documentation, and organizational skills.
Proficiency in using EMR systems and standard office software.
Must have dependable transportation and the ability to travel locally to client homes.
Working Conditions
Primarily field-based with travel to client homes within Suffolk County.
May require occasional office time for meetings, documentation, and supervision.
Regular business hours (MondayFriday, 8:00 a.m.5:00 p.m.) with on-call responsibilities.
