We are seeking a dedicated and experienced Home Health Physical Therapist to join our team. The ideal candidate will possess at least one year of home health experience and will be responsible for providing skilled physical therapy services to our patients in their residences. This is a per-diem position that will transition to a full-time position.
Key Responsibilities
- Conduct thorough assessments and evaluations of patients' physical abilities and needs in a home setting.
- Develop and implement individualized treatment plans based on patient assessments and clinical findings.
- Utilize various therapeutic techniques, exercises, and modalities to improve patients' mobility, strength and overall function.
- Educate patients and their families on rehabilitation techniques, safety precautions, and home exercise programs.
- Monitor and document patient progress, adjusting treatment plans as necessary
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Qualifications
- Master's or Doctoral degree in Physical Therapy from an accredited program
- Current state licensure as a Physical Therapist
- Minimum of one year of experience in home healthcare
- Strong organizational, communication, and interpersonal skills.
- Ability to work independently and manage a diverse caseload within home environments
- Valid driver's license and reliable transportation
