Minimum Requirements

  • Associate Degree in Nursing and four (4) years' experience as a Registered Nurse which includes at least one (1) year of Minimum Data Set (MDS) or Registered Nurse Assessment Coordinator (RNAC) experience (or certification submission required at the time of application).
  • One (1) year minimum people management experience in nursing.
  • An equivalent combination of education, training, and experience is acceptable.

Licensing as a Registered Nurse (RN) in the state of Pennsylvania will need to be provided and verified prior to appointment.

It is the policy of Allegheny County that in order to receive a final offer of employment, candidates must successfully pass a background check, which includes verification of Allegheny County Real Estate Tax status, and drug/medical examinations as required for the position. The pre-employment drug screen must be completed within 48 hours of the acceptance of the conditional offer of employment.

Position Summary

Reporting to the Regional Center Administrator (RAC), The Nurse Assessment Coordinator (NAC) is ultimately responsible for the oversight and coordination of the federally mandated Resident Assessment Instrument (RAI) process, which includes the Minimum Data Set (MDS) assessment, Care Area Assessment, and care plan development, implementation, revisions, and evaluations, along with various other related duties as noted below. The incumbent in this role also supervises Registered Nurse Assessment Coordinator (RNAC) positions.

Overview

Duties:

  • Coordinates the RAI process, which includes, at a minimum, the following elements:
  • Minimum Data Set (MDS);
  • Care Area Assessment process;
  • Care plan development;
  • Care plan implementation;
  • Evaluation.
  • Oversees output of the Registered Nurse Assessment Coordinators who complete assessments and transmit results to the national repository. Reviews final validation reports and corrects or modifies assessments in response to system warnings or errors as needed.
  • Coordinates the completion of comprehensive care plans according to regulatory requirements with case managers for residents utilizing managed care, health maintenance organizations (HMOs), commercial insurance, and other alternate payment models.
  • Maintains the OBRA and PPS assessment schedules.
  • Ensures compliance with state-specific regulations regarding the RAI process.
  • Provides insight and analysis of MDS-based Quality Measures.
  • Serves as a member of the Quality Assessment and Assurance (QAA) and/or the Quality Assurance and Performance Improvement (QAPI) committees.
  • Audits and improves staff education/competency as needed to ensure accurate and timely completion of supporting documentation and MDS assessments.
  • Reviews resident complaints and grievances associated with the RAI process and care delivery protocols; makes written reports of action taken; discusses with the resident, representative, and family as appropriate.
  • Collaborates with interdisciplinary team in determining skilled level of care for Medicare residents and procuring required Medicare-specific documentation. Ensures physician certification/recertification of a skilled level of care throughout the Medicare stay, including involvement in making Medicare eligibility determinations.
  • Also collaborates with interdisciplinary team in discharge planning, training, caregiver training, and the provision of resources as needed. Partners with discharge planners and physicians to obtain accurate and complete documentation to support ICD-10-CM diagnosis coding and surgical procedures.

General

  • Communicates with residents, personnel, and providers over the phone, via email or other written documentation and responds to all inquiries.
  • Follows appropriate safety and hygiene measures at all times to protect residents and themselves.
  • Maintains confidentiality of protected health information, including verbal, written, and electronic communications.
  • Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel. Reports any retaliation or discrimination to HR or compliance officer.
  • Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor/Administrator/Abuse Coordinator. Protects residents from abuse and cooperates with all investigations.
  • Performs other duties as requested or required.

Knowledge Of

Knowledge, Skills, and Abilities

  • Current state and federal regulations, practice standards, and procedures, as well as guidelines pertaining to the RAI process, particularly the following pertinent sections of Appendix PP:
  • 483.20 Resident Assessments.
  • 483.21 Comprehensive Resident-Centered Care Plans.
  • Center for Medicare and Medicaid Services (CMS) regulations.
  • Long Term Care (LTC) and Skilled Nursing Facility (SNF) regulations and guidelines.

Skill In

  • Patient-centered nursing best practices.
  • Accurately and effectively coordinating and administering the Resident Assessment Inventory and properly certifying the results.
  • Electronic health record (EHR) systems.
  • Microsoft Office Suite, including Excel, and basic computing systems.

Ability To

  • Foster effective working relationships and build consensus.
  • Plan, organize, prioritize, work independently, and meet deadlines.
  • Use sound judgment and make independent, effective decisions.
  • Work effectively with individuals at all levels of the organization, as well as with residents, family members, visitors, government personnel, and the public.

  • Learn computer systems, system applications, and related office equipment.
  • Maintain confidentiality of sensitive information.

Residency: Allegheny County within 1 year.
