Summary
This position assists members in their communities to access physical, oral, and behavioral health services by building trusting relationships. This enables the HRSN Navigator to serve as a liaison linking intermediary between the member, various services and the community, improving quality, and cultural competence for services being rendered. This improves capacity by increasing health literacy, knowledge, and self-sufficiency through a range of activities such as outreach, community education, screening, social support, and advocacy.
Essential Duties
- Provides screening, education, information, assistance, tools, and support to enable a member to make the best health care decisions for their unique personal situation, with a focus and prioritization on Health Related Special Needs (HRSN) programming.
- Works in the office making phone calls and occasionally makes visits to member’s homes and/or multiple care settings, as needed.
- Completes all required documentation inclusive of electronic documentation in a timely manner in accordance with regulations and AllCare CCO and Medicare Advantage policies and procedures.
- Participates in the development of individual treatment plans with the member and their primary care team, including supporting members HRSN goals and interventions as appropriate.
- Assists in removing barriers to care and individual self-management goals by bridging community, cultural, linguistic, educational, and other barriers to encourage self- care.
Job Duties
- Assists members and their families in accessing community-based support systems, understanding treatment options, and preventive behaviors.
- Helps coordinate member transportation and accompaniment as needed to scheduled appointments.
- Meets members where they are at, in multiple care settings, as needed to ensure that members are supported in making high value care decisions.
- Anticipates, identifies, and helps members to overcome barriers within the health care system to ensure the prompt resolution of emerging health crises.
- Supports and educates members in social support services, basic health education, provides advocacy, health system navigation and community coordination.
- Participates in regularly scheduled staff development trainings to maintain and improve personal knowledge base of various chronic diseases.
- Participates in and attends regularly scheduled supervision meetings and shares both successes and challenges of working with high-need members and their medical team.
- Proactively communicates all concerns to health care team, PCP and program manager, as necessary, seeking support and guidance.
- Interprets and represents the goals and objectives of the community resource specialist to community members and other agencies.
- Assists in quality initiatives for the department, as needed.
- Coordinates care needs for the Dual Special Needs Population (DSNP) member to ensure compliance with the Special Needs Model of Care (SNP MOC).
- Prepares for and participates in all required DSNP training, education, and member audits.
- Participates in initial, annual, and ongoing trainings.
- Acts as preceptor during the onboarding of new hire employees.
- Maintains punctual, regular, and predictable attendance.
- Works collaboratively in a team environment with a spirit of cooperation.
- Respectfully takes direction from care coordination leaders.
- Meets all required training including those listed in Relias Learning Module System (LMS).
- Other duties as assigned.

This position may have voluntary on- call responsibilities.
Supervisory Responsibilities
This position does not have any supervisory responsibilities.
Job Requirements
- Requires the use of personal vehicle for local travel (subject to mileage reimbursement).

All health care providers who are HIPAA covered entities, whether individuals or organizations, must obtain an NPI number. Under HIPAA, you’re a covered health care provider if you electronically transmit health information in connection with a HIPAA standard transaction, even if you use a business associate to do so. AllCare Health employs staff that are defined under OHA or CMS as health care providers. Positions that are defined as providers include, but are not limited to, Doctors, Nurse Practitioners, Physical Therapists, Traditional Health Workers, and Certified or Qualified Healthcare Interpreters.
Education
High school diploma or general education degree (GED) required.
Experience
- At least three to six months’ related experience and/or training in a similar role required.
Certificates, Licenses and/or Registrations
- Valid Oregon Driver’s License and vehicle insurance.
- Completes the National Provider Identifier (NPI) registration within the first 30 days of completion of the CHW.
- Current Mental Health First Aid Certification or willingness to be certified within 90 days of hire.
- Traditional Health Worker (THW) certification through OHA is required or to be obtained within two years of hire date. (Subject to pay increase upon Certification)
Technical Skills
- Basic medical knowledge, including ability to recognize social and medical risk factors, trauma symptoms and behaviors, and chronic disease conditions.
- Familiarity with the Healthcare industry.
- Exceptional writing, editing, and proofreading skills.
- Excellent computer skills, including the Microsoft Office Suite (Outlook, Word, PowerPoint, and Excel).
- Knowledge of and compliance with HIPAA regulations.
