Hybrid Case Coordinator

We are looking for a compassionate and organized Hybrid Case Coordinator to join our home care team. This role is responsible for assessing client needs, developing care plans, coordinating services, monitoring progress, and ensuring compliance with agency policies and regulations.

Responsibilities

• Conduct client assessments and develop individualized care plans.

• Coordinate home care services, appointments, and communication with clients, families, and providers.

• Complete a minimum of two home visits per month (phone visits may be approved when necessary).

• Monitor client progress and update care plans as needed.

• Maintain accurate documentation and ensure HIPAA compliance.

• Provide emergency direct care coverage when staffing needs arise.

Qualifications

• At least 1 year of healthcare experience (home care or case management preferred).

• Experience working with individuals with disabilities is a plus.

• Current CPR/First Aid certification.

• Strong communication, organization, and problem-solving skills.
