Role Overview
We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.
What You’ll Do
Routine & urgent assessments; chronic disease management
Post-discharge follow-ups, med reconciliation, labs/diagnostics
Coordinate with facility staff, specialists, families/POAs
Same-day documentation; CMS/regulatory compliance
Schedule & Setting
Full-time; weekday schedule. On-site care across Stockton, Modesto, and Amador with clustered routes to minimize travel.
Qualifications
Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
Geriatrics or facility-based experience preferred; EMR proficiency
Compensation
Annual compensation range: $120,000–$160,000 (based on experience, schedule, and patient volume)
Additional earning opportunities available based on coverage and panel needs
W-2 with full benefits available; mileage reimbursed per policy
