Job Summary
We are seeking a compassionate and dedicated Personal Care Assistant to provide essential support to individuals in need of assistance with daily living activities. The ideal candidate will have experience in patient care, particularly within home health environments. This role requires a commitment to enhancing the quality of life for our clients, ensuring their comfort and safety while promoting independence.
Responsibilities
- Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting.
- Provide companionship and emotional support to clients.
- Assist with mobility using equipment such as Hoyer lifts when necessary.
- Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and meal preparation.
- Document care provided in Electronic Medical Records (EMR) systems while adhering to HIPAA regulations.
- Support clients with memory care needs, including those with Alzheimer's or dementia.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Provide hospice care and support for clients with terminal illnesses as needed.

Qualifications
- Previous experience in caregiving, home health, or long-term care settings is preferred.
- Knowledge of medical terminology and experience with medical documentation is advantageous.
- Familiarity with EMR/EHR systems for accurate record keeping.
- CPR certification is required; additional certifications in patient care are a plus.
- Experience working with individuals with developmental disabilities or special needs is desirable.
- Strong observational skills to monitor client health and report changes effectively.
- Ability to perform physical tasks associated with caregiving including lifting and transferring clients safely.
- Excellent communication skills to interact effectively with clients, families, and healthcare teams.
