External Liaison – Allaire Rehab & Nursing

Allaire Rehab & Nursing is seeking a dynamic and relationship-driven External Liaison to join our team. The ideal candidate will be responsible for building and maintaining strong referral relationships with hospitals, physicians, and other healthcare providers to support census growth and ensure smooth transitions of care into our facility.

This is a highly visible role that requires excellent communication, healthcare marketing experience, and a passion for creating trusted partnerships that connect patients with the high-quality care they deserve.

Responsibilities:

• Develop, nurture, and maintain strong relationships with hospital discharge planners, case managers, physicians, and other referral sources.
• Act as the primary external representative of Allaire Rehab & Nursing within hospitals and healthcare networks.
• Educate referral sources about Allaire’s services, programs, and specialized care offerings.
• Collaborate with internal admissions and clinical teams to ensure timely, efficient, and seamless admission processes.
• Monitor referral activity and maintain a consistent presence within assigned hospitals and healthcare systems.
• Identify and address potential barriers to admissions and work to resolve them quickly.
• Conduct regular outreach, presentations, and follow-ups to strengthen community and hospital partnerships.
• Track, analyze, and report referral patterns and market trends to leadership.
  
  
  

Qualifications:

• Minimum 2–3 years of healthcare marketing, liaison, or admissions experience required; Skilled Nursing Facility (SNF) experience strongly preferred.
• Demonstrated success in developing hospital referral relationships.
• Strong communication, presentation, and interpersonal skills.
• Ability to problem-solve and collaborate effectively with both hospital staff and internal facility teams.
• Highly organized with the ability to manage multiple relationships and priorities.
• Knowledge of healthcare referral processes, post-acute care, and insurance requirements.
