Responsibilities:
Provide social work services to patients and residents across our facilities, including residential, long-term, rehabilitation, and subacute care settings
Conduct assessments, develop care plans, and provide counseling to patients and families to address social, emotional, and psychological needs
Collaborate with interdisciplinary team members to develop and implement individualized care plans
Provide education and support to patients and families on disease management, coping strategies, and community resources
Facilitate discharge planning and arrange for post-discharge services as needed
Maintain accurate records and reports, adhering to all regulatory and compliance requirements
Participate in quality improvement initiatives and contribute to the development of policies and procedures
Requirements:
Bachelor's or Master's degree in Social Work (MSW) from an accredited program
Minimum of 3 years of experience as a social worker in a nursing home
Licensure or certification as a Social Worker in the state of New Jersey (LSW or LCSW)
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment and prioritize tasks
Proficiency in Electronic Medical Records (EMRs) and other software applications
