What are the responsibilities and job description for the Hospice Registered Nurse position at All State Care Hospice?
The hospice RN field nurse is primarily responsible for comprehensive nursing assessments and admissions, routine follow up visits, and ensuring appropriate documentation in the patient's electronic medical record. This position enables the patient to receive high-quality end of life care through nursing interventions in accordance with the established plan of care. The hospice RN will work closely within the interdisciplinary team including the physician, RN case manager, medical social worker, certified home health aides (CHHA), chaplain, and bereavement coordinator. In this position, you will be able to create meaningful relationships with both the patient and family members as you guide them through the transition to the end of life.
We are looking for an experienced RN with at least 1 year of hospice experience. A Bachelor's Degree is preferred but is not required. Candidates must have a valid California driver's license and up to date car insurance. A full-time position may be offered after a 60-day probationary period.
All State Care Hospice is ACHC and Medicare accredited hospice agency based in the heart of West Hollywood providing services throughout Southern California.
