All of Us In Recovery (A.I.R.) is dedicated to helping individuals build stability and work toward their personal recovery goals. Our Care Coordinators provide individualized support and help participants connect with resources that can improve their quality of life and independence.
About the Role
We are seeking a motivated Care Coordinator to work with individuals enrolled in our Free Through Recovery and Community Connect programs, with opportunities to work within the 1915(i) Medicaid program as well.
The Care Coordinator serves as a consistent support for participants, helping them identify needs, establish goals, access community resources, and stay connected with appropriate services.
Key Duties
- Meet with participants to identify strengths, needs, barriers, and personal goals.
- Create and maintain individualized service and support plans.
- Assist participants with accessing healthcare, housing, employment, education, and other community resources.
- Coordinate with community providers and other professionals involved in a participant’s care.
- Monitor progress and provide ongoing encouragement and support.
- Help participants build stability and successfully transition into the community.
- Complete required documentation and maintain accurate participant records.
What We're Looking For
- Bachelor’s degree in social work, human services, behavioral health, or a related field OR at least one year of case management or behavioral health experience.
- Current and valid driver’s license.
- Ability to complete basic computer, data entry, and documentation tasks.
- Strong communication, organization, and problem-solving abilities.
- Self-motivated and dependable with a person-centered approach.
- A genuine interest in advocacy and supporting individuals in reaching their goals.
Additional Qualifications
- Completion of required North Dakota training is a plus.
- Familiarity with the criminal justice system, including Parole and Probation, is preferred.
Compensation & Schedule
- Commission-based compensation based on assigned caseload.
- Part-time position with the potential for increased hours as the caseload develops.

Pay: $1,500.00 - $7,000.00 per month
