Essential Job Functions
As an RN Case Manager, you will play a vital role in providing patient-centered care while ensuring clients receive the support they need in their homes and communities.
🔹 Conduct comprehensive assessments and develop individualized care plans.
🔹 Provide nursing care, medication administration, and health education.
🔹 Monitor and reassess patient needs, adjusting care plans accordingly.
🔹 Collaborate with physicians, families, and support staff.
🔹 Supervise PCA/home health aides and mentor new RN Case Managers.
🔹 Maintain accurate documentation and fulfill certification/recertification requirements.
🔹 Participate in on-call duties and quality improvement initiatives.
Requirements
✔ Active Registered Nurse (RN) License
✔ Two-Step TB Test (we provide testing if needed)
✔ DHS Criminal Background Clearance (we provide screening)
✔ Valid Driver’s License & Auto Insurance
