What are the responsibilities and job description for the Referral Specialist – Weekends position at All Care?
The Weekend Intake/Referral Specialist serves as the primary point of contact for referral sources, managing incoming referrals via electronic platforms, fax, and phone. This role reviews and processes clinical referrals across all service lines, supports care coordination for complex cases, and ensures accurate, timely intake documentation to facilitate patient admissions.
Schedule: This position follows a Saturday and Sunday, 9am-5pm schedule.
Work Location: The Weekend Intake/Referral Specialist position is based out of our Lynn office with the option of working remotely every other weekend.
Essential Duties & Responsibilities:
- Receive, review, and process referrals; gather and verify clinical information to support admissions.
- Communicate with referral sources to assess patient and family service needs.
- Ensure accurate documentation and timely communication of referral information to clinical teams.
- Maintain knowledge of medical terminology, home care services, and insurance (Medicare, Medicaid, private).
- Document patient/family concerns and incident reports as needed.
Qualifications:
- Knowledge of medical terminology and home care processes preferred.
- Strong customer service, communication, and organizational skills.
- Proficiency in data entry and ability to multitask in a fast-paced environment.
- Proficiency in EMR/EHR systems
- Proficiency in Microsoft Word, Outlook, and Excel.
