Essential Duties & Responsibilities
- Practice independently in the community as part of an interdisciplinary care team.
- Conduct patient assessments and develop individualized, goal-oriented Plans of Care based on best practices.
- Manage an assigned caseload in accordance with agency productivity and quality standards.
- Coordinate patient care with physicians, caregivers, and interdisciplinary team members to ensure continuity of care.
- Complete timely and accurate clinical documentation, including visit notes, care coordination, telephone communications, and physician orders.
- Perform and document formal and informal case conferences.
- Supervise and coordinate services provided by discipline extenders (LPNs, PTAs, COTAs) and home health aides.
- Monitor patient progress and adjust care plans as needed to support positive clinical outcomes.
- Promote improvements in key quality measures, including hospitalization reduction, mobility, wound healing, medication management, pain control, and respiratory status.
- Valid driver’s license is required.
- Active Massachusetts Registered Nurse (RN) license is required
- Minimum of one year of clinical nursing experience under qualified supervision
- Home health or community health nursing experience preferred.
- Excellent communication and interpersonal skills to effectively educate and support patients and their families.
- Ability to work independently and manage patient care in a home setting.
- Proficiency in using electronic health record (EHR) systems for documentation and communication.
- Problem-solving skills to modify treatment plans based on patient progress and challenges.
- Use of a company vehicle, including paid insurance, maintenance, and gas. (Subject to meeting eligibility requirements.)
