Position Overview
All Care’s Intake Referral Coordinator plays a key role in ensuring timely and accurate processing of referrals and patient information into the agency’s electronic medical records (EMR) system.
This position supports the intake and admissions process by entering patient demographics and health-related data and communicating effectively with referral sources, patients, and internal clinical teams.
Schedule: Monday-Friday, 9am-5pm

Position Overview

Essential Duties & Responsibilities
- Accurately input referral information, demographics, insurance, and clinical data into the EMR system in a timely manner.
- Monitor fax and electronic referrals for new admissions.
- Follow up with hospitals, physician offices, and case managers for missing information.
- Maintain confidentiality and compliance with HIPAA and all other regulatory guidelines.
- Provide administrative support to the intake team as needed, including answering phones, scanning, and filing documents.
- Education:
- High school diploma or equivalent; an associate’s degree is preferred.
- Experience:
- Previous experience in health care, intake coordination, or customer service is preferred.
- Experience with medical terminology is a strong advantage.
