Responsibilities
- deliver professional nursing services in client's home setting and provide case management for primary patients
- complete initial assessment and plan of care for home care clients
- prepare clinical and progress notes, submitting all required paperwork daily
- coordinate services, informing the physician, management, and other personnel of changes in the client's condition and needs
- ensure quality and accuracy of patient records and charts in compliance with Medicare and Medicaid standards
- demonstrate compassion while working with ill, disabled, elderly and/or emotionally disturbed patients
- respond promptly to telephone calls and emails
- participate in continuing education programs to stay current on home health practices, laws and regulations
- post hospital visits
- monthly client visits
- help to maintain confidentiality files
- medication management services
- perform other duties as assigned
- RN with current MA license, BSN preferred
- one or more years of current nursing experience in certified home care is preferred, but not required
- ability to sit, stand, bend, lift, kneel, climb stairs and move patients safely as required
- ability to work in the patient's home environment as well as the office
- demonstrate strong verbal, written and interpersonal skills to communicate effectively
- must have reliable automobile, valid driver's license, and current auto insurance
- fluent in English with skills in a second language a plus
- computer skills sufficient to capture notes and required documentation
