What You'll Do
- Deliver skilled, compassionate hospice care according to each patient's individualized plan.
- Conduct thorough assessments and collaborate with the interdisciplinary group (IDG) to build and adjust care plans.
- Educate patients and families on the end-of-life process, comfort measures, and available support services.
- Monitor patient conditions, report changes promptly, and coordinate adjustments with the attending physician.
- Supervise Hospice Aides and LPNs to ensure consistent, high-quality care.
- Maintain accurate documentation in compliance with all state, federal, and accreditation standards.
- Participate in IDG meetings, quality initiatives, and staff development opportunities.
- Provide insight and emotional support to families, offering calm and guidance when it's needed most.
- Support patients holistically - medically, emotionally, and spiritually - to enhance comfort and quality of life.
What You'll Bring
- Graduate of an accredited nursing program.
- Current Registered Nurse (RN) license in Illinois.Minimum of 1 year of RN experience; hospice experience preferred.
- Excellent clinical judgment, assessment, and problem-solving skills.
- Strong communication and organizational abilities.
- Valid driver's license, reliable transportation, and current insurance.
