Role Description The Case Manager is a full-time, on-site role based in Lawrence, MA, responsible for coordinating and overseeing care services for clients receiving in-home and community-based support. This role includes assessing client needs, developing and updating individualized care plans, and ensuring appropriate services are in place across programs such as home care, adult foster care, and waiver or PACE services. The Case Manager will regularly visit clients, monitor progress, document services, and collaborate with nurses, caregivers, physicians, and external partners to promote safety and well-being. Daily responsibilities also involve maintaining accurate records, supporting compliance with regulatory and program requirements, communicating with families and caregivers, and responding to changes in client condition or support needs. The role requires proactive problem-solving, strong advocacy for clients, and a commitment to person-centered, culturally sensitive care.
Qualifications
- Strong case management and care coordination skills, including assessment, care planning, and ongoing monitoring of client needs.
- Effective communication and interpersonal skills for collaborating with clients, families, caregivers, and multidisciplinary teams.
- Organizational and documentation abilities to manage caseloads, maintain accurate records, and support regulatory compliance.
- Knowledge of home care, adult foster care, waiver, or PACE programs and familiarity with community resources and support services.
- Ability to work on-site in Lawrence, MA, conduct home visits, and adapt to varying home environments and client situations.
- Relevant education such as a degree in social work, nursing, human services, or a related field; professional licensure or certification in case management or social work is preferred.
