- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans.
- Administer medications, injections, IV infusions, and other treatments following strict protocols and medical documentation standards.
- Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile technique procedures.
- Manage patient care in diverse settings such as medical-surgical units, ICU (Intensive Care Unit), emergency rooms, and specialty areas like cardiology or ophthalmology.
- Utilize EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Cerner or Epic to document patient information accurately and efficiently.
- Conduct specimen collection and processing, phlebotomy, diagnostic evaluations, and specimen handling adhering to infection control standards.
- Support discharge planning, case management activities, and coordinate with multidisciplinary teams to ensure seamless patient transitions across care settings.
- Provide specialized care for patients with complex needs such as dementia or Alzheimer's disease, including memory care strategies and behavioral health support.
- Assist in emergency situations by managing airway management, ventilator support, or acute pain management protocols.
- Work with individuals with developmental disabilities or those requiring hospice or palliative care services to deliver compassionate support aligned with their needs.
- Valid LPN license in good standing with the state licensing board.
- Proven experience in diverse healthcare environments such as hospital medicine, long-term care (nursing homes), assisted living facilities, or outpatient clinics.
- Critical care experience preferred, including ICU or Level I Trauma Center exposure; experience with Level II trauma centers is a plus.
- Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for medical documentation and patient records management.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding for procedures and treatments.
- Competence in performing venipuncture (venipuncture), IV insertion/infusion techniques, catheter care, medication administration including injections and botulinum toxin treatments.
- Experience working with special populations such as pediatrics (including neonatal ventilators), geriatrics (including dementia & memory care), behavioral health patients or individuals with disabilities.
- Excellent patient assessment skills combined with the ability to work effectively under pressure in urgent or emergency situations like trauma or infectious disease outbreaks.
- Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times; proficiency in medical imaging procedures is advantageous.
