Habilitation Nurse (Registered Nurse)

Position Summary:

We are seeking an experienced Registered Nurse (RN) for a hybrid, field-based Habilitation Nurse position. This role involves conducting nursing assessments, coordinating services, and advocating for members with developmental and complex healthcare needs.

The RN will conduct in-person assessments statewide every 90 days and as needed, with an assigned core district. Travel is required, and mileage reimbursement is provided.

Responsibilities:

• Conduct nursing assessments and determine medical necessity for skilled nursing services.
• Complete required reports and documentation within established timeframes.
• Monitor members' health needs and assist with referrals and vendor calls.
• Collaborate with members, families/support systems, healthcare providers, and external agencies.
• Participate in multidisciplinary planning meetings and advocate for members' rights and individualized needs.
• Complete special projects, mandatory meetings, and required annual training.
• Maintain accurate documentation using Google Workspace, Microsoft Office, Adobe Acrobat, and other required systems.

Qualifications:

• Active Arizona RN license in good standing without restrictions.
• Minimum 2 years of recent RN experience in home health, hospice, long-term care, or case management.
• Excellent communication, clinical assessment, organization, and documentation skills.
• Ability to work independently and travel throughout Arizona.
• Experience with behavioral health, complex medical conditions, or nursing case management preferred.

• OIG & APS clearance
• CPR/BLS Card
• Required professional license/certification
• Vaccine status documentation
• TB Skin Test
• Hepatitis B vaccination or declination of offered vaccine
