Qualifications:
- Graduate of an accredited Occupational Therapy Assistant program
- Current Texas state license as a COTA
- Two years of experience in Home Health preferred
- Spanish-speaking preferred
Responsibilities:
- Adhere to established agency policies and procedures
- Provide physician-prescribed occupational therapy services under a plan of care established by an Occupational Therapist (OT)
- Help patients improve or maintain their ability to perform activities of daily living (ADLs)
- Support patients in regaining independence and achieving their highest functional potential
- Carry out skilled therapy procedures as ordered by the physician and directed by the OT
- Collaborate with the OT and other healthcare personnel in patient care planning
- Report changes in patient condition and progress to the supervising OT
- Educate patients and family members/caregivers on home programs and ADL strategies
- Participate in mandatory in-service and performance improvement activities as assigned
- Attend patient care conferences as scheduled
- Document timely and accurate visit notes, progress updates, and care plan revisions as per agency requirements
