Responsibilities:
- Conduct thorough MDS assessments, develop, and implement personalized care plans for residents
- Collaborate with interdisciplinary team members to identify and address resident needs, goals, and concerns
- Provide restorative nursing services, including rehabilitation, therapy, and skilled nursing care
- Develop and maintain accurate and detailed records of resident care, progress, and concerns
- Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination
- Participate in quality improvement initiatives to optimize care outcomes and resident satisfaction
- Stay up-to-date with industry trends, regulations, and best practices to ensure evidence-based care
- Minimum of 1 year of experience as an MDS/Restorative Nurse
- Current licensure as a Registered Nurse (RN) in the state of Illinois
- Strong assessment, communication, and problem-solving skills
- Ability to work effectively in a fast-paced environment with high levels of patient acuity
