Job Summary
Will assist in providing comprehensive case management services to the eligible population who struggle with mental health, substance use, or co-occurring disorders, as well as provide support for their families and significant others. Will also make necessary referrals and coordinate services, by promoting linkages between mental health and/or substance treatment services, primary care clinics and other social service providers to ensure that clients have access to all available and necessary services to obtain an optimum level of functioning. Make frequent contact with clients in their natural settings and/or where client is receiving care, to focus on practical problems of daily living, promote assertive advocacy, assistance with transportation if needed.

Job Summary

Essential Duties And Responsibilities
- Conduct comprehensive assessments, intake, and orientation on program participants to determine individual and family needs.
- Assist in the development of service plans based on the identified needs of the client/family.
- Maintain accurate, neat, timely, and appropriate documentation of services provided in client file.
- Utilize Motivational Interviewing and Stages of Change approaches to engage and support clients in meeting their goals towards positive choices and behaviors. Support shall include follow- up and discharge planning during and following intervention services.
- Identify family risk factors and help improve the health and functioning of the family unit by engaging the family members in support and involvement in the intervention/prevention process.
- Make frequent contact with clients through home visits, telephone calls, and office visits to encourage and offer support. Meet with clients and their families on a regular basis to monitor and follow up on their needs and preferences.
- Provide case management services in settings that are based on the needs of the clients and the goals of the client's service plan, including office-based, home-based, or community-based locations, and/or provide client transportation, as needed.
- Provide individual, family, and group education, activities that help reduce the risks of reengaging in substance use or risky behaviors, and crisis interventions.
- Promote and encourage entry into substance use and/or mental health services and make referrals, if appropriate, for clients who need formal treatment.
- Work with the treatment team and partners to remove barriers and assure clients access to essential resources.
- Bachelor's degree in Sociology, Psychology, Social Work, or Human Services.
- Two years of related experience.
