Alinea Family Hospice Care is GROWING!! With that growth, we are seeking to hire a full time licensed hospice social worker for the Rockwall, Quinlan areas!
The primary function of the Hospice Social Worker will be to provide coordinated care to terminally ill patients at their place of residence and/or facility. The position requires advanced education in social work. The social worker must possess knowledge of the Hospice philosophy and culture, value quality, competent care, and understand the general principles of end of life care and death/dying. The social worker must have the ability to support patients and their families with the complex emotional needs associated with terminal-illness, anticipatory grief, and bereavement by analyzing their needs to determine appropriate counseling and providing individual/family counseling, and facilitate communication among patients, family members, and members of the IDG team.
Responsibilities:
- Performs initial psychological, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal directed IDG care plan.
- Conducts ongoing reassessments of patient/family needs and counseling as required.
- Provides short term crisis intervention and individual or family counseling as required.
- Provides services, under the direction of a physician (who approves Plan of Care).
- Participates as a member of the Bereavement Team as assigned.
- Participates as a member of the QAPI Committee.
- Observes, assesses and brings to IDG conferences information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family.
- Assumes the active role of advocate for the patient/family unit.
- Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.
- Documents comprehensive psychosocial, emotional and spiritual assessment clearly and concisely in a timely manner.
- All patient/family visits, telephone contacts and referral actions are recorded in the patient record per policy.
- Provides ongoing counseling related to issues of death and dying to the patient and family as needed.
- Attend all meetings, such as IDG and any other assigned.
- Adheres to all Hospice policies.
- Assumes responsibility for own personal and professional development and maintenance of skills in social work.
- Exhibits Hospice philosophe in all job-related roles.
REQUIRED:
- Master of Social Work from an accredited university (or)
- Bachelor of Social Work from an accredited university with supervision of a Master of Social Work
- One (1) year of social work experience; 1 year of experience in community health setting (hospice, acute care, mental healthcare or home care preferred)
- Able to work with multi-disciplinary team, community agencies, clients family and friends
- Dependable transportation to make community-based home and facility visits
- Fully licensed LMSW, LCSW, or LBSW in the state of Texas
- Valid Texas Driver's License and valid Texas Automobile Liability required
- Experienced computer user, Kinnser knowledge preferred but not required
