On-Site Home Health Director

We are seeking a On-Site Home Health Director opportunity in the Greater Buffalo, NY area.

Salary: $77,833 – $101,899/year full benefits

We are seeking an experienced, hands-on Registered Nurse (RN) to lead daily operations and clinical oversight for a growing home healthcare program serving older adults across two premier campuses.

This is a high-impact leadership role for an RN with strong home health, home care, hospice, or healthcare operations leadership experience who thrives in clinical oversight, team development, and program growth.

Key Responsibilities:

• Lead on-site daily home health operations and clinical services

• Ensure compliance with state, federal, and regulatory standards

• Drive quality outcomes, patient satisfaction, and service excellence

• Oversee staffing, hiring, training, coaching, and retention

• Manage care coordination, scheduling, and interdisciplinary communication

• Support program growth through referrals and community partnerships

Qualifications:

• Active New York RN license required

• CPR & First Aid required

• Strong leadership experience in home health/home care preferred

• Proven ability to manage both clinical and operational performance
