ORGANIZATION OVERVIEW
The Ali Forney Center (AFC) is dedicated to protecting LGBTQ youth from the harms of homelessness and empowering them with the tools needed to live independently. Founded in 2002, AFC provides comprehensive services, including housing, healthcare, and educational support to over 2,000 youth annually. AFC's mission is to create an affirming and supportive environment where LGBTQ youth can thrive.
POSITION OVERVIEW
The Intensive Case Manager is responsible for providing substance use screenings and assessments, mental health referrals, treatment planning, advocacy, escorts, and coordination of services for homeless clients who are suffering from co-occurring disorders (substance abuse and mental illness).
KEY RESPONSIBILITIES
- Conduct substance abuse screenings and assessments and develop treatment plans for AFC clients.
- Provide ongoing intensive case management services (crisis counseling, outreach, referrals, advocacy, escorts, and discharge planning) to clients with co-occurring disorders.
- Perform evidence-based practices as assigned and written documentation of services provided.
- Facilitate regular occurring support groups at the Drop-In Center.
- Participate in weekly treatment planning meetings and provide knowledge and expertise about substance issues to direct care staff.
- Some weekend and evening hours required.
- Attend weekly staff meeting and supervision and perform other duties as assigned.
