Instructions:
1) Complete and submit this application; List 3 professional references within your application and push-out the electronic surveys. 2 satisfactory references must be returned in order to be approved.
2) Upload a letter of recommendation from your University/Program indicating the area of required observation and number of hours needed.

3) Complete an online Volunteer Background check through our system at this link: Background CheckOnce these steps are completed, you will receive notification from our Human Resources office further instructions, such as fingerprints process, approval, and placement options.
All Clinical/Internship students must complete a fingerprints credentialing clearance thru TEA.
