We are looking for a committed, caring, and reliable Admissions Coordinator to join our in-house rehab team for our Skilled Nursing Facility!

RESPONSIBILITIES:
Ensuring the seamless admission process for our patients.
Assist with patient transfers and arrivals to the facility, scheduling and ensuring a positive experience for all involved, follow up on referrals, verify insurance and other required authorizations, and general office duties.
Coordinate with healthcare professionals to receive referrals and schedule admissions
Conduct patient assessments to determine eligibility and appropriate level of care
Collaborate with insurance companies to ensure approval and coverage for admissions
Maintain accurate and up-to-date patient records and documentation
Communicate and coordinate with interdisciplinary team members to facilitate smooth transitions and admissions
Schedule and conduct facility tours for prospective residents and their families
Provide excellent customer service to patients, families, and healthcare professionals
REQUIREMENTS:
High school diploma or equivalent; Bachelor's degree preferred
Good customer service and a team player
Interest in working with the geriatric population
Strong organizational and time management skills
Excellent communication and interpersonal skills
Knowledge of admission procedures and regulations in skilled nursing facilities
Familiarity with medical terminology and documentation
Proficiency in computer applications and software
Ability to work both independently and collaboratively in a fast-paced environment
Ability to effectively communicate and collaborate with residents, families, and healthcare professionals
Detail-oriented and able to multitask effectively
Compassionate, empathetic, and patient-centered approach to care
EXPERIENCE:
A minimum of 2 years of experience in healthcare or related fields with excellent customer service, communication skills, and detail-oriented are welcome
Experience working in a skilled nursing or long-term care facility
