Job Overview
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our healthcare hospice team. The ideal candidate will provide high-quality nursing care to patients in various settings, in their homes, assisted living facilities or other settings. This role requires strong clinical skills, the ability to work collaboratively with healthcare professionals, and a commitment to patient-centered care.
Responsibilities
- Administer medications and treatments as prescribed by physicians.
- Monitor patient vital signs and report any significant changes to the healthcare team.
- Assist in the development and implementation of patient care plans.
- Provide education and support to patients and their families regarding health management.
- Perform basic life support and emergency response as needed.
- Conduct assessments of patients’ conditions, including tube feeding and catheterization.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Participate in health coaching initiatives to promote wellness among patients.
Requirements
- Current Licensed Vocational Nurse (LVN) license in the state of CA
- Knowledge of anatomy and basic nursing procedures.
- Strong communication skills and the ability to work effectively within a team environment.
- Commitment to providing compassionate care and advocating for patients’ needs.
- Ability to manage multiple tasks efficiently while maintaining attention to detail.
- Minimum of 1 year LVN experience; hospice or palliative care experience preferred
- Ability to work independently and with flexibility.
- Strong communication and documentation skills
- Valid driver's license and reliable transportation
- CPR certification, BLS
Schedule:
- Per Diem, flexible hours
- Weekends and evenings may be required based on patient needs
