We are looking for an engaged and energetic registered nurse to join our diverse provider team. We are looking for someone who has worked in a family practice/primary care setting and has experience working in a street outreach or community health settings. They would be adept in screening and prioritizing clients. We need a candidate that has general experience in all types of wound care and would prefer someone certified for this specialty.
The Registered Nurse (RN) position provides professional nursing care for patients in the medical clinic and on outreach in the Bernalillo County service area. The RN will also consult and collaborate with other cross-disciplinary staff on issues relating to the physical and mental health of patients.
We comply with all OSHA and CDC guidelines regarding PPE and have various procedures in place to safeguard employees.
AHCH is currently hiring a Registered Nurse for the 24/7 Gateway Medical Respite Center 5400 Gibson SE. This is a co-location role (most of the time at respite, meetings and limit shifts at the clinic downtown) This position may require some nights and weekends as needed. Hours align with the regular campus schedule, 7:30am-4:00pm, Monday-Friday.
Duties and Responsibilities:
- Work with other disciplines toward integrative approaches to patient care
- Establish a compassionate environment by providing biopsychosocial support to patients
- Manage the care of patients in collaboration with other health professionals and transdisciplinary teams
- Serve as a cross-department clinical resource for AHCH staff
- Collaborate with other staff in treatment planning
- Participate in planning, case review and quality assurance/quality improvement activities
- Participate in peer review process
- Coordinate continuity of care for AHCH patients hospitalized and through medical respite upon release
- Maintain standards of medical care according to established protocols
- Ensure timely documentation of services in EHR in compliance with AHCH standards
Minimum Qualifications:
- Ability to thrive in a complex healthcare setting
- Have a strong desire to work in an integrated/cross-disciplinary team model
- Experience documenting services in an EHR
- Knowledge of diagnostic and treatment room procedures
- Experience with health promotion, education, and maintenance
- Knowledge of medications and their effects/side effects on patients
- Ability to assess problems quickly and accurately and find appropriate solutions
- Must have excellent communication skills and have ability to educate clients on health-related topics
- Must be able to work with individuals from a wide variety of backgrounds and life experiences in a respectful and culturally competent manner
- Graduate of a School of Nursing program
- Current RN license in New Mexico or eligibility to obtain RN license in New Mexico
- Experience working in a primary or urgent care clinical setting
Preferred Qualifications:
- Bachelor’s Degree of Science in Nursing (BSN)
- 2 years of experience working in a primary or urgent care clinical setting
- Previous experience with outreach/street medicine
- Clinical experience with underserved populations, those with SMI/CMI, and substance abuse disorder
- Previous experience working in a FQHC or community health setting
- Bilingual Spanish/English
