Seeking a bi-lingual Consortium and Outreach Coordinator to join the Bronx Healthy Start Partnership team. The Bronx Healthy Start Program aims to address gaps in birth outcomes in Bronx County, through the provision of direct support to families during pregnancy and postpartum and engaging partners in the broad mission of achieving birth equity with a special focus on the social determinants of health. The goal of the position is to facilitate and support a coalition of partners to address the social determinants of health and achieve systems changes that improve birth outcomes.

Under the direction of the Project Director, the Consortium and Outreach Coordinator operates cooperatively as part of a multi-disciplinary team to convene and facilitate a robust Community Consortia (formerly known as Community Action Networks or "CANs") comprised of diverse multi-sector partners that advise and inform BxHSP activities, as well as develop and implement action plans to improve perinatal outcomes within Bronx County. The Consortium and Outreach Coordinator supports the development of culturally responsive referral partnerships to address diverse participant needs, collaborating with healthcare providers, mental health services, and organizations focused on housing and food insecurity and stakeholders such as Title V, public health departments, substance abuse agencies, community leaders, and service agencies.

Assist the Project Director and staff in a supportive role to ensure program operations and functions are upheld.

Position Responsibilities

Responsibility Area: Consortium Coordinator

Under the direction of the Project Director:

  • Identify, engage, and maintain relationships with organizational partners to sustain and expand investment in birth equity initiatives;
  • Convene and facilitate multi-sector collaborations involving state, local, and community stakeholders, including Bronx Healthy Start participants.
  • Based on feedback from consortium members, recommend and implement strategies for providing direct services to Bronx Healthy Start participants.
  • Develop and manage cross-sector partnerships to ensure access for Bronx Healthy Start participants for coordinated, comprehensive maternal, child, and family medical care; health and parenting education; and community-based resources that address social determinants of health within the project area.

Responsibility Area: Community Engagement and Planning

  • Participate in Communities of Practice with other Healthy Start projects and local partners to develop and execute a strategic plan adressing social determinants of health.
  • Collaborate with consortium members to define performance measures aligned with strategic goals.
  • Lead planning and execution of Consortium activities, ensuring timely coordination of logistics, outreach, and materials

Responsibility Area: Communication

  • Draft and maintain communication tools include Partner Information Packet and referral agreements.
  • Collaborate with Communications staff to ensure the development of engaging health education, media, and social media content.
  • Represent Bronx Healthy Start at community events such as health fairs to promote visibiity and recruit clients.

Responsibility Area: Data Management/Reporting

  • Oversee data collection and documentation of partnership activities, including tracking key indicators and demographic breakdowns.
  • Maintain a comprehensive database of partners and stakeholders.
  • Prepare monthly activity summaries and contribute to required program reports.
  • Administer and analyze annual membership surveys to inform program improvements.

Other:

  • Assist with special projects including proposal writing, and/or mentoring of other team members as assigned.
  • Perform all other duties as assigned.

POSITION SPECIAL RESPONSIBILITIES:

  • Must be available to work occasional evenings and weekends.
  • Must be able to organize and attend regular events and training.

Qualifications

  • Undergraduate degree in a related discipline and a minimum of four years of related work experience or a minimum of six years of related work experience.
  • Knowledge of and passion for systems change and collective impact.
  • Skilled facilitator of groups and meetings
  • Excellent listening and documentation skills
  • Ability to set agendas, motivate stakeholders, and navigate conflicting viewpoints.
  • Interest in women's health, including prenatal and post-partum concerns; mental health, childbirth, infant development, and local community and social service resources.
  • Knowledge and respect for the socio-cultural experiences of the populations served.
  • Fluent in English and Spanish (read, write, speak) is required.
  • Specialized training in childbirth, doula, postpartum doula, or lactation education is a plus.
  • Experience with virtual and in-person event planning, including meetings, training, and webinars.
  • Detail-oriented with effective communication, presentation, and people skills.
  • Ability to build relationships with diverse stakeholders.
  • Ability to collect, analyze and manage data.
  • Excellent communication skills, including ability to engage diverse stakeholders and develop compelling narrative and visual content.
  • Proficiency in computers and use of MS Word and Excel, Zoom required.

Minimum Salary Range

USD $65,000.00/Yr.

Maximum Salary Range

USD $70,000.00/Yr.
