Provide case management as a best practice ensuring excellence in service and documentation in all phases including screening, assessment, stratifying risk, planning, implementation, follow-up, transitioning, communicating post-transition, and evaluation. Case managers participate as an interdisciplinary team member in both staffing and consultation to ensure the continuum of care for essential needs as well as mental health.

Minimum Education and Experience:
- Bachelor’s degree in social work.
- Two (2) years of directly related social work experience including case management.
- Master’s degree in social work or LMSW.
- Five years of experience in social work.
