Job Summary:
The Linkage Navigator is responsible for connecting individuals at risk of HIV/STI/HCV with prevention services and resources. This role involves providing education, support, and linkage to care, ensuring that clients receive timely and appropriate services to reduce their risk of acquiring infection.
Duties & Responsibilities:
Client Engagement:
- Conduct outreach to identify and engage individuals at high risk for HIV/STI/HCV
- Conduct risk assessments and develop personalized prevention plans for clients
- Establish rapport and build trusting relationships with clients
- Follow up with clients to ensure they are connected to and retained in care
Education & Counseling:
- Provide education on prevention methods, including PrEP (pre-exposure prophylaxis), PEP (post-exposure prophylaxis), and safer sex practices
- Address patient concerns and misconceptions, offering emotional support throughout the process
- Stay updated on the latest HIV prevention strategies and resources
Navigation & Coordination:
- Assist clients in navigating the healthcare system to access testing, prevention, and treatment services
- Develop and maintain relationships with healthcare providers, community organizations, and other stakeholders to facilitate referrals and service coordination
Data Management:
- Maintain accurate and confidential records of client interactions and services provided
- Compile and analyze data related to HIV/STI & HCV testing, diagnosis, and treatment outcomes
Qualifications & Skills:
- Knowledge of HIV/STI/HCV transmission, prevention strategies, and treatment.
- Strong communication and interpersonal skills with the ability to deliver critical program information at the clients’ level.
- Ability to work with diverse populations.
- Analytical skills to assess and resolve complex socio-economic issues effectively.
- Proficiency in Microsoft Office and data management software.
Education, Licensure & Experience:
- Bachelor’s degree in Public Health, Social Work, or a related field (or related experience).
- 1 years of experience in HIV and/or STI prevention.
- Experience in navigating healthcare systems.
- Experience in HIV prevention, health education, or case management.
Work Schedule:
- Regular schedule Monday to Friday 8AM - 4:30PM
- Ability to work flexible hours, including evenings and weekends, as needed.
Work Location:
- All Agency locations, In person.
- Travel within the community to various outreach sites.

Requirements:
- Must maintain a valid driver’s license and current automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive company owned vehicle or your personal vehicle during company business.

Salary : $39,520 - $43,680
