Job Purpose

This position is currently grant-funded through per-member-per month payments tied to contracts between Medicaid, insurance companies, and local health departments. At this time, funding is secured through December 31, 2026.

Join our team as the Lead Nurse Coordinator for the Care Management for At-Risk Children (CMARC) program, serving children ages 0–5 in Alamance County who are at increased risk for developmental delays or health challenges due to prematurity, congenital conditions, toxic stress, or complex family environments. In this leadership role, you will provide comprehensive, family-centered care management services to eligible children and families while guiding the daily work of two CMARC Social Workers through mentoring, caseload assignment, and annual performance evaluations. Working closely with the Care Management Nursing Supervisor and the multidisciplinary Care Management Team, you will help shape program operations, strengthen community partnerships, and promote an optimal level of functioning for each child by ensuring high-quality, coordinated, family-centered care.

Program Focus

The CMARC program coordinates medical, nutritional, educational, social, and emotional services to ensure children receive the support needed for a healthy start. Care Managers work closely with private providers and community partners to strengthen family stability and promote long-term well-being.

Responsibilities

  • Care Management Services — Conduct assessments, develop care plans, coordinate services, facilitate referrals, and provide ongoing evaluation for children and families.
  • Family Support — Offer counseling, advocacy, and guidance to help families navigate medical, educational, social, and emotional needs.
  • Community-Based Engagement — Provide services in homes, physician practices, and community settings to meet program goals and Medicaid requirements.
  • Electronic Documentation — Utilize multiple electronic medical records and state systems to document care and track program outcomes.
  • Program Quality & Reporting — Collect and analyze program data, contribute to performance improvement, and assist with policy and procedure updates.
  • Team Collaboration — Support onboarding and training for new staff and students; collaborate with nursing leadership and medical directors.
  • Hospital-to-Home Transitions — Provide care management for hospitalized Medicaid-enrolled children ages 0–5 as they transition back to the home environment.
  • Public Health Response — Assist with outbreak investigations and emergency response activities as needed.

Qualifications

Associate degree required; supplemented by two years of experience as a registered nurse.

Special Requirements

  • A current license to practice as a registered nurse (North Carolina by the NC Board of Nursing is preferred).
  • Valid N.C. driver's license and have access to a personal vehicle.

  • Computer Proficiency — Adequate computer skills for communication, electronic documentation, and accessing patient information systems.
  • Nursing Expertise — Thorough knowledge of nursing theory, practice, and techniques used in public health, including obtaining health histories and performing physical assessments to identify deviations from normal.
  • Resource Coordination — Considerable knowledge of available community resources and organizations, with the ability to coordinate services based on patient and family needs.
  • Social & Economic Awareness — General understanding of current social and economic issues affecting public health, including health disparities and their impact on families and young children
